How to qualify for AHCCCS through the Baby Arizona process

Pregnant women often are able to qualify for "Baby Arizona" without going through the entire AHCCCS qualification process through to DES (Department of Economic Security).

Usually, Nurse-Midwife offices can do the process for you right at their office and if you bring in all the requested documentation you can get an approval the same week if you qualify. Baby AZ is VERY easy and fast.

Income guideline$:
  • Single applicant who is pregnant: allowed income is up to $1,712/mo.

  • Applicant living with a parent or spouse 2/3 of the time: combined allowed income of the parties: $2,147.

  • Applicant living with both parents 1/2 of the time: combined allowed income is $2,582.

  • Income allowance is increased $489.00 a month per additional child in the family.

  • Income is net - take home income after taxes.

If you can do the Baby AZ application through the midwife's office you will need to make an appointment and bring in the following documentation in order to submit an application.
  • Picture ID — one of the following:
    Driver's license, AZ official ID, Military ID, School ID


  • Proof of U.S. Citizenship — one of the following:
    Birth Certificate, passport, baptismal certificate or
    US Tribal ID


  • Certificate of U.S. naturalization


  • Social Security card (or at least your Social Security number)


  • Proof of Residency — one of the following:
    • Lease agreement or mortgage coupon that shows your address
    • Driver's license that shows current address
    • Voter Registration card that shows current address
    • A utility bill, rent receipt, bank statement or letter from landlord as proof of your current address


  • Proof of Earned Income:
    • If single: All pay stubs for the last 30 days
    • If married: All pay stubs for you and your husband for the last 30 days
    • If the pregnant woman is less than 18 years old and is living at home the parents income will be counted. If she is over 18 and NOT living at home the parents income is not counted.

      Proof: you and your parent's pay stubs for the last 30 days.


  • Proof of Unearned Income:

  • Unearned income is money received from any source other than work such as social security benefits, child support, loans, gifts etc.

  • Medical Insurance card or a copy of the policy if you have other insurance.
